Coordinated Services Team (CST)
*Please note that CST is a program provided through Family & Children’s Unit however referrals may be made through Behavioral Health due to similarity in program goals and service delivery methods.
Goals of the Program
CST provides a structured, personalized team planning process consisting of the family, natural supports, and providers. A plan of care is developed with the purpose of meeting the child and family’s needs within the community.
Eligibility
- Age 5-18
- Child is at risk of out-of-home placement
- Other interventions have not been successful over time or significant obstacles to services access
- Child involved in at least 2 of the following: Mental Health, Special Education, Youth Justice, Child Protection, or AODA programming
- Parent(s) and child willing to be actively involved in the teaming process.
