Children's Long Term Services (CLTS, also known as Waiver) and Children's Community Options Program (CCOP)

Goals of the Program 

  • Offers services to meet the individual disability-related needs of child(ren) with physical, developmental and/or mental health disabilities, and their families, with services that promote inclusion in the community and accessibility to their environment. Families partner with Support & Service Coordinator to discuss the needs and goals to create an individualized service plan. Some service categories include: support services, teaching & skills development, management & coordination, physical aids and housing related needs.  

Eligibility 

  • Age 0-21 

  • Determined functionally eligible by screening tool 

  • Wisconsin resident, US citizen, or acceptable immigration status 

Funding 
  • Wisconsin Medicaid 

  • Federal – Centers for Medicare & Medicaid Services 

  • State Government
