Community Support Program (CSP)

Program Information

  • CSP is a voluntary community-based program. CSP services occur in the community with a primary goal of connecting clients with community resources.

  • CSP assists persons living with chronic mental illness to access treatment, rehabilitation, and support services in the community.

  • CSP offers a case manager and an internal team for ongoing assessment and services.

  • Range of services include assistance with advocacy, coordination of care and resources, crisis intervention, psychiatric symptom management and nursing, psychotherapy, employment services, socialization (individual and group based), and skills development.

Eligibility 

  • Individuals age 18+

  • Resident of Dunn County, currently residing in the community or planning for transition to a community setting

  • Clinically determined to be eligible based on mental health assessment and mental health diagnosis

  • Functionally eligible as determined by Wisconsin State Functional Screen

  • Presence of mental health and/or substance symptoms impacting daily functioning in areas such as independent living, social interactions, interpersonal relationships, and vocational activities.

Funding 

  • Medicaid, Long Term Family Care or a variety of private insurances are accepted.

  • At times a co-pay/ deductible is required based on ability to pay.

  • CSP serves those that are uninsured or underinsured.
