How To Apply
- Visit ACCESS to complete an online application. You will still need to complete an interactive interview
- Call the Great Rivers Call Center at 1-888-283-0012 and complete your application and interview over the phone. Call center hours are Mon, Tues, Thurs and Fri from 8am-4pm and Wed from 8am-11am.
- Visit Dunn County Department of Human Services and apply in person
- Complete a paper application and return it one of 3 ways:
- Fax it to 1-855-293-1822;
- Mail it to CDPU, PO Box 5234, Janesville, WI 54547-5234; or
- Bring it to Dunn County Department of Human Services
It is important that you start your application as soon as possible. This will be the start of your benefits if you are eligible. The start date is the date the agency receives your name, address and signature (electronically, by phone or by paper application). The sooner you apply the more benefits you may receive for the month. You will have 30 days to complete the full application process.
