Comprehensive Community Services (CCS)

CCS is a recovery focused integrated behavioral health program for youth and adults with severe mental health and/or substance use disorders.  Individual and Service Facilitator create an individualized recovery plan and team to support and facilitate recovery and/or stability.  

Mental health and/or AODA needs affect day-to-day functioning such as in work, school, family, job, legal, etc. 

Lives in community setting 

Determined functionally eligible by screening tool 

Determined clinically eligible by mental status exam and determination of need 

All services provided through Wisconsin Medicaid
